All workplace accidents should be reported on and incident reports kept for five years. Your Hazard Register should be updated and steps
taken to ensure the accident does not occur again (if possible).
Above you will find a Sample Template for an Accident & Incident Report Report Form. You should familiarise yourself with this form, amend it to suit your business (if required) and print a copy to keep in your Hazard Register so the form can be completed immediately after the accident or incident.
